Patient Financial Responsibility
Policy
You, as a patient of Freiberg Orthopaedics and Sports
Medicine, are responsible for all of the fees which you incur as
a result of being our patient. We accept insurance assignment
and will file your insurance claim(s) for you. You are
responsible for all co-payments and/or balances as required by
your insurance plan. You must bring your insurance card(s) for
all visits to Freiberg Orthopaedics. Your appointment will be
rescheduled if you do not bring your insurance card(s) with you.
You must obtain all referrals required by your insurance company
from your primary care physician prior to your visit to us. We
do not secure referrals for our patients. All co-payments and
co-insurance are due at the time of your visit. A patient who is
younger than eighteen (18) must be accompanied by an adult who
is responsible for his/her co-payments, co-insurance, and/or
deductibles.
Acceptable Methods of Payment
Freiberg Orthopaedics and Sports Medicine accepts all of the
following methods of payment – cash, check, bankcard (debit
card), and credit cards (Visa, Master Card, and Discover Card).
Pre-Certification for Elective Surgery
Pre-certification by your insurance company is required in
advance of all elective surgeries scheduled with us. Our staff
will contact your insurance carrier for required approvals. All
co-insurance and/or deductibles must be paid in advance of your
surgery.
